Becoming a 4-H Club Leader

4-H Club Leader

Becoming a 4-H club leader is an immensely rewarding volunteer experience. Youth need caring, supportive adults in their life, and you can truly make a very positive difference in the lives of your club members. As a 4-H club leader you will organize club meetings, direct the project areas your club chooses, and provide guidance to the members in the club.

There are three steps in the application process that each volunteer will need to complete. All 4-H volunteer leaders will need to fill out all appropriate forms and undergo a background check (finger prints are not required & no cost for volunteer) and pay the registration fee of $1.00 ($2.00 for horse and livestock). Volunteers cannot hold or participate in club meetings until volunteer leader applications are into the county 4-H office and background check is ran and approved. Once the volunteer’s background check has come back cleared, the county office will call the volunteer and send an acceptance email welcoming them to the 4-H team and explaining the next steps the volunteer will need to take.

Step 1: New volunteer leaders will need to fill out the following forms:

BCI Waiver-Background Check

Salt Lake County/ 4-H Volunteer Application

4-H Leader Registration Form

Step 2: Turn in forms to the county office along with registration fee.
The office is located on the 1st floor, South Building of the Salt Lake County Government Center, Suite S1200:

USU Extension, 4-H
2001 S. State Street
Suite 1200
P.O. Box 144575
Salt Lake City, UT 84115-4575

Step 3: Acceptance phone call and email from the 4-H county Office
